ORAL HEALTH IN SENIOR CITIZENS

As people grow older certain health concerns need our full attention especially the oral health.

Good oral health is an integral part of the overall health.

Old age shows number of oral health problems :

1) Periodontal disease:

Poor oral hygiene or high levels of dental plaque are associated with severity of periodontal disease.

Gum disease worsens if there is poor fitting dentures and in certain diseases such as diabetes, cancer.
Periodontal disease is a leading cause of tooth loss.

2) Tooth loss:

Severe dental caries and periodontal disease are the major reasons for the tooth loss.

3) Denture related lesions:

Denture stomatitis is the inflammation of tissues underlying the dentures.
Ill fitting dentures, improper usage of denture and poor oral hygiene causes denture stomatitis.

Denture hyperplasia is the overgrowth of tissues under the dentures.

Traumatic ulcers seen with overextended dentures or unbalanced occlusion .

4) Root caries:

Tooth roots are more prone to decay.
In periodontal disease the roots will be exposed outside because of gum recession, as a result root caries come on the exposed root.

5) Dry mouth:

Dry mouth/Xerostomia comes from ageing and by usage of some medications.
Dry mouth causes dental caries and chewing,eating & communicating difficulties.
Smoking is another risk factor of dry mouth.
Dry mouth is mostly seen in patients treated for hypertension, psychiatric or urinary problems.

6) Bone resorption:

Bone resorption results from tooth loss and not replacing a missing teeth.

Oral health maintenance in older people:

1) Increasing oral hydration.
2) Brushing twice daily.
3) Flossing once daily.
4) Avoid tobacco and alcohol.
5) Every six months once dental checkup.

Conclusion:

Oral health impact will be there on general health and the quality of life.
Poor oral health can increase the risk to general health and affects the nutritional intake.

DENTAL TRAUMA

Dental trauma refers to trauma(injury) to the teeth and/or periodontium and nearby soft tissues such as lips, tongue etc.,

Dental injuries:

Enamel fracture --

                

Enamel-dentine fracture -- uncomplicated crown fracture.

Enamel-dentine fracture involving pulp exposure -- complicated crown fracture.

Root fracture of tooth --

  

Periodontal injuries:

Subluxation -- tooth knocked loose.

Lateral luxation -- lateral displacement of tooth.

Extrusive luxation -- external displacement of tooth.

Intrusive luxation -- internal displacement of tooth.

Avulsion -- tooth knocked out.

Injuries to supporting bone:  

Injury involves the alveolar bone and may extend beyond the alveolus.

Risk factors for dental trauma:

1) Age - especially young children , mostly 2-3 years of age.

2) Sports, especially contact sports such as football, hockey, basketball, cricket.

3) Military training.
4) Class II malocclusion with increased overjet and class II skeletal relationship with  incompetent lips.

Prevention:

Prevention is nearly impossible to stop accidents from happening, especially in children who are quite active.

Regular use of a gum shield during sports and other high risk activities, is the most effective prevention for dental trauma.

Dental management during pregnancy.

PREGNANCY - DENTAL MANAGEMENT

The hormonal changes during pregnancy causes changes in the mother's body. 

Increase in secretion of female hormones and foetal growth causes several systemic, physiological and physical changes in pregnant women.

Oral changes seen in pregnancy include gingivitis, gingival hyperplasia, pyogenic granuloma and salivary changes.

First trimester (1-12 weeks)

• Educating the patient about maternal oral changes which occur during pregnancy.
• Strict oral hygiene instructions to be given.
• Limiting the treatments to oral prophylaxis and emergency treatments.
• Radiographs to be avoided.

Second trimester (13-24 weeks)

• Oral hygiene instructions to be given.
• Scaling, polishing and curettage can be performed if necessary.
• Emergency dental care to be given.
• Radiographs to be avoided.

Third trimester (25-40 weeks)

It is safe to perform dental treatment in the early part of 3rd trimester. Pregnant women position in dental chair to be left lateral position and not to be supine position.

• Oral hygiene instructions to be given.
• Scaling, polishing and curettage can be performed if necessary.
• Better to avoid dental treatments during second half of 3rd trimester.
• Radiographs to be avoided.

                  An old male patient came to our clinic with pain in upper and lower front teeth region. He also complains of long term missing teeth in upper and lower back teeth region on both sides. He also had difficulty in chewing the food.

 As there is long term missing teeth, the opposing teeth near missing teeth region got supra-erupted (drifted towards missing teeth space), the bite got collapsed and the facial height is reduced, so to bring back the normal height of his face & to correct the bite, we need to create space for replacing the missing teeth, so we advised to go for root canal treatment and crowns for opposing teeth.

As there is only minimal tooth structure left for lower front teeth, feasibility to save the teeth is not possible, we advised to go for removal of lower front teeth.

Since the patient is not interested to go for implants, replacement is done by flexible denture and precision attachment and bridges.
